Application Process
For new applications, the following documents are required:
- AB-200 Alcohol Beverage License Application
- AB-101 Alcohol Beverage Appointment of Agent (Corporations/LLCs)
- AB-100 Alcohol Beverage Individual Questionnaire (for the Agent and all members of the Corporation/LLC)
- Proof of Responsible Beverage Server Course or proof of licensure in the past two years (for the Agent)
- Copy of Wisconsin Seller’s Permit
- Description of the operation (narrative or plan of operation)
- Due Date: at least 15 days prior to the granting of the license
For renewal applications, the following documents are required:
- AB-200 Alcohol Beverage License Application
- AB-100 Alcohol Beverage Individual Questionnaire (for the Agent)
- Copy of Wisconsin Seller’s Permit

Who needs a retail alcohol beverage license?
A person, firm, partnership, corporation, or association within the City of Wauwatosa needs a license to sell, barter, or exchange fermented malt beverages or intoxicating liquor, in any quantity whatsoever.
Types of Licenses
"Class A" Liquor License: For sale of intoxicating liquor in original packages or containers for off-premises consumption only. "Class A" Liquor License Application.
Class "A" Beer License: For sale of beer in original packages or containers for off-premises consumption only. "Class A" Beer License Application.
"Class B" Liquor License: For sale of intoxicating liquor by the glass for on-premises consumption, and in the original package or container in multiples not to exceed four liters at any one time to be consumed off the premises where sold. "Class B" Liquor License Application.
Class "B" Beer License: For sale of beer for on-premises or off-premises consumption. "Class B" Beer License Application.
"Class C" Wine License: For sale of wine by the glass or in an opened original container for consumption on the premises where sold. "Class C" Wine License Application.
Temporary Class "B" Beer and "Class B" Wine (Picnic) License: For sale of fermented malt beverages or wine at a particular picnic or similar gathering of limited duration. Issued only to bona fide clubs, to fair associations or agricultural societies, to churches, lodges, veterans' posts, chambers of commerce, or similar civic or trade organizations or societies that have been in existence for at least six months.
Qualified organizations may obtain not more than two temporary "Class B" wine licenses in a 12-month period.
The following forms are required for temporary Class "B" beer and "Class B" wine licenses:
- AB-220 Temporary Alcohol Beverage License
- AB-101 Appointment of Agent
- AB-100 Individual Questionnaire (Agent only)
- The appointed Agent must provide proof of completion of a Responsible Beverage Server Course, or proof of licensure in the past two years
Application materials must be submitted at least 10 days before the event.
Alcohol Licensing Fees
Liquor Licenses (Annual Fees)
- "Class A" & "Class B" Liquor License: $500.00
- Class “A” Beer License: $300.00
- Class “B” Beer License: $100.00
- "Class C” Wine License: $100.00
Other Annual Fees
- Background Check: $15 per year
- Publication Fee: $8 per license per year
Temporary / Event-Related Licenses
- Temporary Class B (Beer and Wine) License
- $10 per event (beer, wine, or both)
- $15 background check fee
- Temporary Extension of Licensed Premise: $75 per event
Administrative Fees
- Change of Agent: $10 application fee + $15 background check fee
- Liquor License Transfer: $10
Change of Agent
If the appointed agent changes, please notify the City Clerk's Office and submit the AB-100 Alcohol Beverage Individual Questionnaire, AB-101 Alcohol Beverage Appointment of Agent, and proof of completion of a Responsible Beverage Server Course, or proof of licensure in the past two years. The Change of Agent fee is $10, plus a $15 background check fee. The forms can be submitted via mail or email to tclerks@wauwatosa.net.
Change of Officer
If the members/officers of a Corporation/LLC change, please notify the City Clerk's office and submit an AB-100 Alcohol Beverage Individual Questionnaire for each new member/officer. There is a $15 background check fee for each new member/officer. This form can be submitted via mail or email to tclerks@wauwatosa.net.
Temporary Extension of Licensed Premises
The Common Council may grant approval to a license holder to temporarily extend the licensed premises for a special event. To apply, go to the Self-Service Portal and submit the application online. Please apply at least ten weeks before the event.
